Should you require a paper application, please submit a written request for such to the board office at 3388 Brentwood Drive, Baton Rouge, La 70809. Your request should include the full address which you would like the paper application mailed.
All applications for renewal and reinstatement may also be submitted through the online services portal by logging into your existing account.
Practitioners: This category includes licensed health care practitioners with prescriptive authority for controlled substances, including physicians [MD] (including those with telemedicine permits [MDT]), podiatrists [DPM], medical interns and residents [INT], dentists [DDS], optometrists [OD], advanced practice registered nurses [APRN] (including those with telehealth permits [RNT]), physician assistants [PA-C], medical psychologists [MP], and veterinarians [DVM].
Facilities: This category includes manufacturers, distributors, third party logistics providers, hospitals, substance abuse clinics, registered outsourcing facilities, laboratories, and hemp growers.. It does not include pharmacies.
Selected Persons: This category includes researchers, medical sales representatives, drug detection canine handlers, and animal euthanasia technicians.
