Application Process Transparency
RS.LA:49:1308 requires internet publication of certain information concerning permits and licenses including; a description of each license or permit issued, full text of current applications, a checklist of all items required to complete the application process, name / contact information of the agency representative responsible for responding to inquiries about the status of an application, anticipated timelines for approval an application, and copies of annual legislative reports as required. Pre-Application Eligibility Determination
Criminal history may be used as a basis for denial for any application. All factors outlined in LA:R.S. 37:2950 may be shall be considered.
In accordance with LA:RS.37:33, an individual convicted of a crime may request at any time, including before obtaining any required education or training, that an entity issuing licenses to engage in certain fields of work pursuant to state law determine whether the individual's criminal conviction disqualifies the individual from obtaining a license issued or conferred by the licensing entity. Facilities
All applications for facility licensing are now available through the Online Services Portal. Please read the following information carefully before you begin. Do not submit applications for facility licensure through the account of an individual.
Please remember that online accounts, just like the permit or license, are location and ownership specific. This means you will need to register a new account for each location you are applying for a credential. If the facility has had a change of ownership, a new account will be required.
When applying for an AMS or EDK, you must register an account for the location housing these devices.

Individuals
All applications for individual licensing are now available through the Online Services Portal. Please read the following information carefully before you begin.
Are you a returning applicant or licensee?
If you have ever applied for a credential with the Louisiana Board of Pharmacy, you already have a username and password even if you have never logged in. Please use the provided "Forgot User ID" and/or "Forgot Password" links, in that order, if you do not know your login information. If you are unable to retrieve your username and password, you may email licensing@pharmacy.la.gov. Do not create a new account.
Are you a new applicant?
If you are a new applicant, meaning you have never applied for a credential with the Board, you may register a new account by clicking “Register”. A verification email will be sent to you after all information is submitted.

Paper Applications
Applications for new credentials are available by electronic format (online). To preview an application and review the application checklist visit the Application Process Transparency page.
