Credentialing Processes
Initial Licensing Process
Pharmacy Benefit Manager (PBM) firms which administer, develop, maintain, perform, or provide any of the pharmacy practice activities identified within the PBM Licensing Law, more specifically at R.S. 40:2868, and must seek to obtain a PBM permit from the Board of Pharmacy by completing the credentialing procedure identified here.
The application for a PBM permit is electronic in nature. You may preview the electronic application at Application Process Transparency prior to starting.

Renewal Process
PBM permit holders will receive a renewal notice by email approximately 60 days prior to the bi-annual expiration date. This notice will include the following guidance.
Online Renewal
Visit the Board’s website, www.pharmacy.la.gov, and select “Login”. Review the information provided carefully then navigate to the start link. Enter your User Id and password previously established. Once logged in select "Online Services" then "Renew".
If you do not know your password select the option "Forgot Password". If you are unable to retrieve your password, please email licensing@pharmacy.la.gov with your updated email address. Please DO NOT REGISTER A NEW ACCOUNT.
Paper Renewal
If you prefer to renew by paper, please use the link below to obtain the paper renewal. Credentials renewed with a paper application will be processed within 2 weeks depending on the volume of paper applications received. Please mail the paper form to the Board office with all required attachments and fees.
