If you require a written verification, beyond what is available using the online lookup, you may submit your request to our office, at the address below, with the required $20.00 fee.

Credential Rosters

The Board now offers credential rosters through our online services portal. These rosters will provide public information on credentials. Rosters are $150.00 each. To obtain a credential roster please follow the directions provided below.

Access your online services account on the Board's Online Services Portal to your online services account. New users should follow the instructions for establishing a new account.
Select Generate Roster
Generate any roster you wish to obtain
Pay the invoice
Download the roster(s) you have generated and paid for
