This category includes manufacturers, distributors, third party logistics providers, hospitals, substance abuse clinics, and registered outsourcing facilities.  It does not include pharmacies.

New CDS Licenses 

The Application for Controlled Dangerous Substance License, for both persons and facilities, is available by electronic format (online).

Should you require a paper application, please submit a written request for such to the board office at 3388 Brentwood Drive, Baton Rouge, La 70809. Your request should include the full address which you would like the paper application mailed.

Renewal of CDS Licenses 

Renewal of the license, for both persons and facilities, is available by electronic format (online) by clicking "Login". The paper application is made available should you require such.
