Application Process Transparency - Pharmacy Technician Candidates
Pharmacy Technician Candidate — an individual, registered by the board, training to become a pharmacy technician, who assists in the practice of pharmacy under the direct and immediate supervision of a Louisiana-licensed pharmacist.
Attention Out of State Technicians:
If you have held a pharmacy technician credential in another U.S. jurisdiction for a minimum of one year and that credential is currently active, please visit Application Process Transparency – Certified Pharmacy Technicians for further information. If you have not held an out-of-state credential for one year or if your credential is not active, please see below.

Applicants for Pharmacy Technician Candidate Registration shall meet one of the following eligibility options:
Option A: At least 18 years of age and enrolled in a nationally-accredited and board-approved pharmacy technician training program (roster of board-approved training programs available on the board website, at www.pharmacy.la.gov). The program director will notify the Board of enrollment.
Option B: At least 16 years of age and enrolled in a Louisiana Department of Education program offering pharmacy technician as a career path. Students must provide their Louisiana Secure ID (LASID) for enrollment verification. You may obtain this number from your school or school system. The Louisiana Department of Education requires this information to verify your enrollment. Without this number, your application cannot be processed.
Option C: At least 18 years of age and can provide verification from a Louisiana licensed pharmacist, who is not on probation with the Board, indicating their intention to provide technician training in a pharmacy that is not on probation with the Board. The training pharmacist must complete Form 034 ~ Pharmacist’s Certification of Training Intention for Pharmacy Technician Candidate Applicant for submission with this application. Required Attachments (Checklist):
- Proof of Identity - Birth certificate, passport, or naturalization papers. Documents not in English must be translated by a professional translating service.
- Social Security Card - Social Security Card.
- Name Change Documents - Legal documents for any name changes that have occurred.
- Photo - Passport style (head and shoulders only) photograph of the applicant.
- Form 33 & Verifications - Applicants that have ever held a professional credential issued by another jurisdiction are required to complete 033 ~ Professional Credentials Disclosure Form . For each license listed, verification of licensure in another jurisdiction is required.
- Prior Actions - If the applicant provided an affirmative response to any questions regarding prior actions:
- Letter of explanation.
- Certified copies of documents from relevant court or government agency.
- Electronic Business Statement - Statement attesting that it is the applicants wish to conduct electronic business with the Board. This statement can be written or typed but must be signed by the applicant.
- Option B Applicants - Your Louisiana Secure ID (LASID) number is required for enrollment verification. You may obtain this number from your school or school system. The Louisiana Department of Education requires this information to verify your enrollment. Without this number, your application cannot be processed.
- Option C Applicants - The training pharmacist must complete Form 035 ~ Pharmacist’s Certification of Training Intention for Pharmacy Technician Candidate Applicant for submission with this application. You may obtain this form at https://www.pharmacy.la.gov/page/forms-technicians.
Criminal Background Check: The board will supply the applicant with information necessary to perform a criminal background check (CBC) with the Louisiana State Police (LSP) and the Federal Bureau of Investigation (FBI). The CBC will be waived for applicants that have previously completed the requirement through this agency.
Criminal history may be used as a basis for denial. All factors outlined in LA:R.S. 37:2950 shall be considered.
Board Review: A review of prior actions and criminal history, is completed when all documents have been received and payment processed.
Issuance: In the event that there are no matters preventing the immediate issuance, the board will issue the credential within fourteen business days of the final requirement being fulfilled. The registration shall expire 24 months after the date of issuance, and it shall not be renewable.
