Application Process Transparency - Pharmacy Technician Certificate
Pharmacy Technician - an individual, certified by the board, who assists in the practice of pharmacy under the direct and immediate supervision of a Louisiana-licensed pharmacist.

As an exception, if you have held a pharmacy technician credential in another U.S. jurisdiction for a minimum of one year and that credential is currently active, please continue below.
Applicants must be at least 18 years of age and non-impaired.
Fee: $100.00 to be paid with the submission of the online application.

For Pharmacy Technicians holding a credential in another U.S. jurisdiction:
- Out of State: If you have held a pharmacy technician credential issued by another U.S. jurisdiction for a minimum of one year and that credential is currently active, Form 33 will be accepted as evidence of education, training, or experience. Further details regarding this form are outlined in the checklist below.
For applicants that received the PTC PRIOR to November 20, 2024:
- Evidence of Training and Experience - Based on option selected for issuance of the Pharmacy Technician Candidate Registration (PTC), proof of practical or past experience must be documented on forms supplied by the board.
- Option A: If you obtained your PTC based on proof of enrollment in a nationally-accredited and Board-approved training program, proof of completion will be submitted to the Board office directly from your training program. This proof of completion will serve as evidence of your required hours.
- Option B: If you obtained your PTC based on your examination and high school education, evidence of at least 600 hours of practical experience earned in compliance with Board rules is required. The forms required for documenting hours for Options B can be found at here.
For applicants that received the PTC on or AFTER to November 20, 2024:
- Evidence of Training and Experience - Based on option selected for issuance of the Pharmacy Technician Candidate Registration (PTC), proof of practical or past experience must be documented on forms supplied by the board.
- Option A: If you obtained your PTC based on proof of enrollment in a nationally-accredited and Board-approved training program, proof of completion will be submitted to the Board office directly from your training program. This proof of completion will serve as evidence of your required hours.
- Option B: If you obtained your PTC based on your enrollment in a Louisiana Department of Education Jump Start program offering pharmacy technician as a career path, proof of completing the program in required. Further, evidence of at least 600 hours of practical experience earned in compliance with Board rules is required. The forms required for documenting hours for Options B can be found at here.
- Option C: If you obtained your PTC based on proof of a Louisiana licensed pharmacist's intent to train, a completed Form 035 ~ Pharmacist's Certification of Training Completion is required as evidence completing the training and experience hours.
ALL applicants for Pharmacy Technician Certificate:
Required Attachments (Checklist):
- Proof of Identity - Birth certificate, passport, or naturalization papers. Documents not in English must be translated by a professional translating service.
- Social Security Card - A legible copy of the social security card is required.
- Name Change Documents - Legal documents for any name changes that have occurred.
- Photo - Passport style (head and shoulders only) photograph of the applicant.
- Form 33 & Verifications - Applicants that have ever held a professional credential issued by another jurisdiction are required to complete Form 033 ~ Professional Credentials Disclosure Form . For each license listed, verification of licensure in another jurisdiction is required.
- Electronic Business Request - Statement attesting that it is the applicants wish to conduct electronic business with the Board. This statement can be written or typed but must be signed by the applicant.
- Prior Actions - If the applicant provided an affirmative response to any questions regarding prior actions:
- Letter of explanation
- Certified copies of documents from relevant court or government agency
- Examination - Proof of successful completion of one of the required examinations:
- A PTCB certificate dated 2000 or later
- ExCPT certificate dated 2018 or later should be submitted.

Criminal Background Check: The board will supply the applicant with information necessary to perform a criminal background check (CBC) with the Louisiana State Police (LSP) and the Federal Bureau of Investigation (FBI). The CBC will be waived for applicants that have previously completed the requirement through this agency.
Criminal history may be used as a basis for denial. All factors outlined in LA:R.S. 37:2950 shall be considered.
Board Review: A review of prior actions and criminal history, is completed when all documents have been received and payment processed.
Issuance: In the event that there are no matters preventing the immediate issuance, the board will issue the credential within fourteen business days of the final requirement being fulfilled.
