Pharmacy Renewal and Reinstatement
The renewal period for pharmacy credentials is November 1 through December 31 annually. This includes pharmacy permits, controlled dangerous substances licenses, and compounded sterile preparation permits.
Credentials that are not renewed by December 31 of each year shall expire and be null and void. The holder of an expired permit and/or license may submit an application, complete with supporting documentation, for reinstatement to the board. Each credential requires a separate application.
Instructions for renewal and reinstatement are outlined below.
Online
Online renewals are available November 1 at 12:00 AM through December 31 at 11:59 PM.
Visit the Board’s website, www.pharmacy.la.gov, and select “Login”. Review the information provided carefully then navigate to the start link.
All Louisiana licensees have an established User ID for the Online Services Portal. Please do not register a new account. You must use your established account to access your renewal. Enter your previously established User Id and the password. If you do not know your password, select the option "Forgot Password". If you cannot reset your password, you may email licensing@pharmacy.la.gov to reset it. Include the pharmacy name, credential number(s), as well as your name and title. You cannot reset your password by calling the Board office. The same instructions here will be the guidance provided when calling.
Once logged in select "Online Services" then "Renewal". Those seeking reinstatement of an expired license should select “Reinstate” from the Online Services Menu after logging in.
Paper
If you prefer to renew or reinstate by paper, visit the Pharmacy Forms and Applications page to select the appropriate renewal or reinstatement application. Mail the completed application and appropriate fee to the Board office. Renewal applications not received or post marked by December 31 will be returned, unprocessed and reinstatement will be required.
You may also mail a written request for any application to the Board office at 3388 Brentwood Drive, Baton Rouge, La 70809. Include your name, license number, full mailing address, and type of application needed.
