Pharmacy Permanent Closure

In the event the owner of a pharmacy contemplates the permanent closure of a pharmacy, there are specific rules of procedure designed to protect consumers' rights of access to their prescription records. You are strongly encouraged to review these rules and assure compliance with all requirements.

 

Permanent Closure of In-State Pharmacy 

Applicable regulations for in-state pharmacy closures can be found in Section 1133 (Pharmacy Closing Procedures) of board regulations  here.

Permanent closure requires two written notices to the Board office – a preliminary notice at least 10 days prior to the closure date, and a final notice after all closure procedures have been accomplished.

  • Preliminary notice shall be submitted to the board at least 10 days prior to the anticipated date of closure.
  • Final notice shall be submitted to the board with all required attachments when the closure procedures have been completed.

Notices may be submitted by logging into the online services account for the pharmacy and selecting "Change of Credential Status or Authority". You are encouraged to review the information provided carefully. Click HERE to start.

Permanent Closure of Non-Resident Pharmacy 

Applicable regulations for non-resident pharmacy closures can be found in Section 2317 (Nonresident Pharmacy Closure Procedures) of board regulations  here.

Permanent closure of a nonresdient pharmacy requires one written notice to the Board office – a preliminary notice at least 10 days prior to the closure date.
