Change of Employment
Credentialed Pharmacy Personnel
A pharmacist, pharmacy technician, pharmacy intern, or pharmacy technician candidate shall notify the board, in writing, of any change of employment within 10 days as defined by Louisiana Administrative Code.
Please note that the previously used paper form has been retired and is no longer available.
Employment changes must be submitted through the Online Services Portal. To do so, log into the established user account, select Online Services, then Employment Change.
The online module will allow licensees to report employment status at a Louisiana permitted pharmacy. If your employer is not a Louisiana permitted pharmacy, you may upload a written statement advising of your employment by selecting Document Upload.
Your request will not be accepted or processed if any of the following apply:
- You are attempting to remove an employer pharmacy which you are currently serving as Pharmacist-in-Charge (PIC). You are required to first complete a change of PIC form. Please visit https://www.pharmacy.la.gov/page/change-of-pharmacist-in-charge for further information.
- You are attempting to add employment using a credential that is cancelled, expired, inactivated, suspended, or revoked. Please assure that you select the appropriate credential when logged into online services account your account.
- You did not provide an inactive date when removing an employer.

Pharmacy Representatives
A credentialed employee will not be included in the list of current employees for a pharmacy permit if the Board has not received a notification of employment change from that licensee.
Notification of employment change may be completed by the licensee as outlined above. If a licensee has obtained a new credential [i.e. a pharmacy technician candidate (PTC) becoming a certified pharmacy technician (CPT) or a pharmacy intern (PNT) becoming a pharmacist (PST)], the licensee must notify LBP of the employment for the new credential. Employment does not transfer from one credential to another.
The Louisiana PIC may remove employees from the current list through the Online Services Portal. To do so, log into the established user account, select Online Services, then Employment Change. The instructions provided should be followed carefully. Please note that a PIC may not add employees.
Your request will not be accepted or processed if any of the following apply:
- You are attempting to remove an employee currently serving as Pharmacist-in-Charge (PIC). You are required to first complete a change of PIC form. Please visit https://www.pharmacy.la.gov/page/change-of-pharmacist-in-charge for further information.
- You are attempting to add an employee.
- You did not provide an inactive date when removing an employee.
