In particular, those pharmacists and pharmacies renewing their credentials for 2012 should take note of this holiday schedule and plan accordingly. All pharmacist licenses, pharmacy permits, and pharmacy CDS licenses - regardless of the date of issue - expire at midnight on December 31, 2011. There is no grace period, and there is no authority to practice with expired credentials. Renewal applications personally delivered to the Board office after December 29, or bearing a postal service postmark after December 31, should be accompanied with the appropriate late fees indicated on the application form.
