BE AWARE OF IMPERSONATION SCAMS: Board staff and NABP (National Association of Boards of Pharmacy) will not contact you by telephone and demand personally identifiable information or payment of funds for any reason.
Description of the Scam - In this scam, a pharmacy receives a phone call from someone who claims to represent the Louisiana Board of Pharmacy. The caller ID shows a Board of Pharmacy telephone number or NABP’s official telephone number, which bad actors have spoofed to appear legitimate. The caller then attempts to solicit information from pharmacy staff, including information that may constitute personally identifiable information (PII). Caller ID spoofing is a technique used by scammers to make an incoming call appear to originate from a trusted or official source. A familiar phone number on caller ID does not confirm the identity of the caller.
Pharmacy Relocations
Relocation of Pharmacy
In the event a relocation is planned, the permit owner shall complete the appropriate application form at least 30 days prior to the planned move.
Please submit your application for relocation by logging into the Online Services Portal and selecting “Address Change / Facility Relocation”.
Should you require a paper application, please submit a written request for such to the board office at 3388 Brentwood Drive, Baton Rouge, La 70809. Your request should include the full address which you would like the paper application mailed.
In-state pharmacies: Upon receipt and review of your properly completed application and payment of the $200.00 fee, Board staff will provide instructions for scheduling your site inspection with your Compliance Officer. After completing the required site inspection for the in-state pharmacy, the Board will re-issue the permit, using the same permit number and the new address.
Nonresident pharmacies: Upon receipt and review of your properly completed application and license verification, issued by the resident licensing authority with the new address, we will update your address on file. There is no fee for a relocation of a nonresident pharmacy permit.
As a reminder, the board no longer issues paper credentials. To verify that your new address is reflected accurately, please complete an online verification for your facility.
