Pharmacy Inspections: Nonresident

In accordance with Louisiana Board of Pharmacy laws and regulations a pharmacy must submit information required by the Board in order to maintain a Louisiana pharmacy permit.  The Louisiana Board of Pharmacy requires that all pharmacy permits have an inspection on file that was completed within the 24 months prior to renewal each year.

Permitted pharmacies will not be eligible to renew the Louisiana permit until an inspection has been received and reviewed by the board dated within the 24 months preceding the opening of the renewal period.   NABP inspections are accepted in place of the state board inspection. You are encouraged to contact NABP if your pharmacy does not have a state inspection within the 24 months prior to the renewal period.

Pharmacies providing sterile and/or non-sterile compounded preparations to Louisiana residents must submit an NABP VPP inspection report, state inspections using the NABP VPP inspection blueprint, or an ACHC inspection report.

If you are unable to provide one of these inspection reports for sterile and/or non-sterile compounding but wish to continue dispensing other prescription medications to Louisiana residents, you may upload a statement attesting that pharmacy will no longer dispense compounded preparations to Louisiana residents.

Inspection reports must be uploaded to the pharmacy account using the Online Services Portal on our website.
