BE AWARE OF IMPERSONATION SCAMS: Board staff and NABP (National Association of Boards of Pharmacy) will not contact you by telephone and demand personally identifiable information or payment of funds for any reason.
Description of the Scam - In this scam, a pharmacy receives a phone call from someone who claims to represent the Louisiana Board of Pharmacy. The caller ID shows a Board of Pharmacy telephone number or NABP’s official telephone number, which bad actors have spoofed to appear legitimate. The caller then attempts to solicit information from pharmacy staff, including information that may constitute personally identifiable information (PII). Caller ID spoofing is a technique used by scammers to make an incoming call appear to originate from a trusted or official source. A familiar phone number on caller ID does not confirm the identity of the caller.
Application Process Transparency - EDK Permit
Emergency Drug Kit (EDK): for long-term care facilities or other board-approved sites, other than a hospital, means a drug
kit containing designated emergency drugs which may be required to meet the immediate therapeutic needs of a resident or patient.
Fee: $50.00

Required Attachments (Checklist): La. Dept. of Health License for the hosting facility.
Board Review: Board staff will verify that one of the following eligibility requirements have been met:
- Hosting facility is one of the following as evidenced by the La. Dept. of Health License:
- A long-term care facility
- An institutional facility without an institutional pharmacy
- Other board-approved site, other than a hospital.
Criminal history may be used as a basis for denial. All factors outlined in LA:R.S. 37:2950 shall be considered.
Issuance: In the event that there are no matters preventing the immediate issuance, the board will issue the credential within fourteen business days of the final requirement being fulfilled.
